What's wrong with this picture?

These are actual photographs from different inspections in our county. Can you see what the problems are?

This is a frustrating sight when you go into a busy place and see these sloppy operating procedures. The cutting board (a food contact surface) is not only dirty with food debris on the cutting surface but on the sides as well showing that it hasn’t been cleaned properly for a while. It is stored on the floor, which is a very dangerous practice. The shelving, the plates and floor all have a build-up of old food/debris. Additionally, aluminum foil is not allowed to line shelves.
