Hello. I did a search and found a very detailed booklet from Los Angeles Public Health Department, called Food Safety Tools For Success. Take a look. It is from another state (California) so you will see a different scoring system and terminology, and the rule that gloves must be worn.

It is full of so many good ideas! Lots of forms that you can print and use. There are examples of what the filled out form would look like too, so helpful. If you have any questions about information in the food safety booklet, please send us an email at mcfoodsafety@multco.us.
